Case Management

Medistaf employs health care professionals with dynamic backgrounds to provide top quality patient services. From initial referral to the rendering of actual patient care, Medistaf’s team of clinical and non-clinical personnel work together to provide comprehensive case management services while, if necessary, coordinating patient care with third party providers.

We support the patient’s overall homecare experience by providing referrals to community health resources, coordinating infusion pharmacy services, and coordinating discharge planning. At the same time, we keep an open line of communication with the patient’s physician or medical provider in order to ensure our patient's general wellness.

Medistaf provides these services in a number of case management settings, consistent with not only the needs of our patients, but those of our Major Accounts as well. These services include:

• Disease State Management, focusing on the entire process rather than just the immediate problem.
• Census driven nursing and case management services to contractual referral sources, providing continuity of care with a single point of contact.
• Professional consulting aimed at streamlining home health operations for various entities to form a cohesive provider team.
• Population based case management and high-tech therapies which respond to a number of factors such as age for the pediatric and elderly population and geographical location, for patients experiencing limited or poor access to medical providers.
